Who We Serve

The Latino Cancer Collaborative provides a culturally and linguistically competent continuum of preventive community education and cancer support services for Latinos and their families in Contra Costa County, California. We work with local, regional and national partners and health care systems to remove barriers to cancer-related services and care and reduce cancer inequities for Latinos.

"As a public hospital where 33 percent of our clients are Spanish speak­ing, frequently with young children and minimal sup­port, they all desperately need these services."

Services

All our services and programs are provided in Spanish by bilingual, bicultural staff.

The free or low cost programs include:

• Community awareness campaigns and screening events
• Community health promoter program with cancer focus
• Cancer education workshops
• Health system linkage and navigation
• Case management and coordination
• Medical interpretation and one to one patient education
• In-hospital emotional support
• Therapeutic support groups
• Cancer treatment education classes
• Advanced care counseling and end-of-life services
• Other support such as emergency financial assistance

LC4 maintains a uniform intake and referral process that assesses client’s needs and connects them to appropriate services within the LC4 network and to resources available in the community including financial support. Patient advocates/case managers are available to assist in the process.

To refer clients to LC4 for support:

• Contact our toll-free General Referral/Information Line at (866) 654-9960 to speak to an advocate/patient care manager
• Fax the Referral Form to our patient advocate/case manager

• Contact any of the LC4 partners for assistance
